Order Forms

Instruction for the COOP order form: (Print if you need)
The order form button is at the bottom of the page.

Open the link below to open up a COOP order form in an Excel Document
Type your name on form at the top
SAVE - It is read only so save it as COOP_YourName (please put your name in place of your name!!:) Save often so you don't loose information.
Starting with page #1, enter your order information. An example is provided. (Call Theresa if you have questions!)
If you run out of room on sheet #1, use the tabs at the bottom of the page to click on page #2. Tabs are labeled "Page 1", "Page 2", "Page 3"
IMPORTANT: Page #3 should be used ONLY for partial orders. DO NOT PUT PARTIAL ORDERS ON PAGE 1 OR 2. Example: You want 1/2 case of paper, or 3 out of 10 boxes of paperclips. An example has been included on Page 3.
When finished - SAVE. (It should already be saved from step #3!)
Print for your records
E-mail back to Theresa. IT WILL NOT SEND UNLESS YOU HAVE SAVED IT!
PC Users use Ctrl+Shift+M and it will open your e-mail program with this in it to send. Type in Theresa's e-mail and send! You are done.
Mac Users Go to File>Send to>Mail Recipient (as Attachment) and your e-mail program will open with this in it to send. Type in Theresa's e-mail OR Mitzi's e-mail and send! You are done.

Reminder to order all supplies you think you will need for the coming year. The items Theresa orders are intended for office use. Theresa will order bulbs for the overhead projectors in the school. Mrs. Luedtke orders Technology items. If there is something special you want, check with her first to make sure she isn't ordering some for the school.

Please do as little "partial" ordering as you can. It's very time consuming and in some cases, I am unable to do it anyway. Maybe you can work with another teacher who uses the same items and split cases with them or something.
